  • Specific: "add alert on burn rate" not "improve monitoring".
  • Measurable: success criteria defined.
  • Owned: a person/team and a due date.

Failure Modes

  • Postmortems as paperwork: no changes, same incident repeats.
  • Too much narrative: not enough concrete prevention work.
